You are required to furnish either of the below for each child at he beginning of the school year.

Immunization Certification Form 

An exemption form

Medical Exemption Form
OR
Non-Medical Exemption Form 

Please note: If you plan to file a nonmedical exemption form, you must watch the Online Educational Module. At the completion of the module, you will be able to complete an electronic form to obtain a Certificate of nonmedical exemption. You can complete this form for more than one child without having to retake the course.

BCD Preschool Immunization Reporting

The state of Colorado requires that schools report their immunization and exemption rates annually. During the 2024-2025 school year, our percent immunized rate for MMR was 90.9% and our percent exempted was 9.1%.

BCD Elementary & Middle School Immunization Reporting

The state of Colorado requires that schools report their immunization and exemption rates annually. During the 2024-2025 school year, our percent immunized rate for MMR was 92.3% and our percent exempted was 7.7%. We did not have any students who were non-compliant.
