IMPORTANT INFORMATION REGARDING IMMUNIZATIONS. Please click here to view important updates from the Colorado Department of Health regarding the necessary immunizations for children in childcare - preschool and click here for those children in grades K - 12. Please note that you are required to furnish a current immunization record, or the appropriate exemption form, for your child or children at the beginning of the 2011-12 school year.
ALLERGY / ASTHMA / MEDICATION INFORMATION. Students with severe allergies who require an Epi-Pen for emergency use at school are required to have an Allergy and Anaphylasix Action Plan in place with the school. Click here for the form which must be completed by you and your health care provider. If your child is also diagnosed with asthma and requires an inhaler at school, the Colorado State Asthma Care Plan needs to be completed as well - click here for the form. Regrettably, we cannot accept any medication without the required authorization/paperwork.
If your child will carry his or her own inhaler or Epi-Pen, the appropriate contract must be completed by you and your child and provided to the school. Please click here for the Student Carry Inhaler Contract or click here for the Student Carry Epi-Pen Contract.
If your child requires any additional medication at school, a separate medication permission form signed by both parent and health care provider is required. Please remember a separate form is required for each medication. Please click here to download and print the Medication Administration form. Again, please remember that we cannot accept any medication without the required authorization/paperwork.

Udi's School Lunch Program Information The February menu is now available; click here to view. Ordering deadline for February meals is January 31st. Please note, due to parental feedback, Udi's will no longer add mayo to its regular sandwiches. Teachers will be provided with packets of mayo for those students who would like to have it on their sandwiches.
